Q:  What time do I need to arrive for my exam?

 

A:  Please arrive 20-30 minutes before your scheduled exam.  This will give you time to complete any necessary paperwork before you see the doctor.

 

 

Q:  What should I bring with me?

 

A:  Please bring a current form of legal identification, such as a Driver's License, Photo ID card, or Passport. This information must be provided for your appointment. You can prepare in advance by Confirming Your Appointment online and requesting a New Patient form.

 

 

Q:  Should I bring my medications?

 

A:  Yes.  Please bring all medications you are currently taking.  This knowledge will provide valuable information regarding your current treatment path with your physician(s).

 

 

Q:  Should I bring my medical records?

 

A:  Please bring any medical records that have not already been submitted previously. Every piece of information is useful in regards to your application.  Actual MRI/CT/X-ray films cannot be officially reviewed, but please bring official interpretations of those films if you have them.

 

 

Q:  What if I'm running late for my appointment or need to reschedule?

 

A:  Please call our toll-free number at 866-212-2212 to update us with any changes that may affect attendance of your scheduled appointment. Depending upon our calendar, we might be able to fit you in later that day, or we can reschedule for another day and time.

 

 

Q:  How do I get directions to the office?

Q:  Do I need to bring money or some form of payment?

 

A:  It is not necessary for you to bring payment at the time of your appointment.
